Living Well Central Oregon

The Living Well with Chronic Conditions program in Central Oregon is a collaborative regional initiative, made possible through the cooperation, dedication, and support of many agencies, organizations and medical facilities in the three-county region. HealthMatters of Central Oregon played the role of neutral convener to bring the various parties to the table in an effort to create a coordinated system to promote the public's health.

The Living Well—Central Oregon initiative aims to meet the needs of those in our three counties who have chronic conditions and limited access to care for their conditions. This initiative offers a team-based provider relationship that teaches real-life skills for living a full, healthy life, and empowers patients with tools to strengthen their health.

The program, based on Stanford University's evidenced-based "Chronic Disease Self-Management Program," is intended to provide more effective care and at the same time reduce the total cost of care.
