The foundation of HealthMatters of Central Oregon is a Health Services Hub that serves as a central clearing house and aims to coordinate the care people receive and to ensure that people's biological, psychological and social needs are met. The Hub does not support any single agency. HealthMatters works in partnership and collaboration to strengthen and to eliminate duplication within the entire health and human services system.

Links 4 Health

The intent of Links 4 Health is to assure a coordinated system that links patients to comprehensive medical and social services, which include: primary and preventive care, specialty and tertiary services as needed, mental health, oral health, vision care, and social services. Links 4 Health : Health Integration Project

This collaborative, following the Pathways model, includes Central Oregon's regional hospital system, safety net clinics, private practitioners, school districts, local health departments, social service providers and consumer advocacy organizations with a focus on behavorial health. Trails to Health

HealthMatters' regional wellness program is adapted from a national evidence-based model designed by HPN Worldwide, a leader in wellness and health consumerism. It empowers employees and families with information, skills and resources for better health and health care throughout their lives. The program is designed to (1) help employees and their families promote health and prevent illness and make informed decisions, and (2) support employers to better manage health costs and rate increases, reduce absenteeism and related costs, and improve productivity and morale. |

Each of these initiatives focuses on improving appropriate, affordable and efficiently delivered primary and preventive services through public-private partnerships. Short-term steps are included in each initiative; and are intended to move Central Oregon toward a well-organized health and human services system that is sustainable.
Although these initiatives are intended to increase availability and access to needed services, ultimately our collaborative work is about ensuring a coordinated system that promotes the public's health throughout the entire three-county region. |
