Our Initiatives

Over the next 3 years,we will begin laying a foundation that will allow for future long-term transformation. This foundation will include specific short-term initiatives designed to bring diverse and competing stakeholders together to begin working on a shared community vision of improving the health of more people at less cost.

Our four initial strategies are:

Medical Home for Children and Youth with Special Health Care Needs –
Integrated Services for the Region

Core Objectives
Establish a pilot medical home model for children and youth with special health care needs (based on American Academy of Pediatrics Model).

Establish care coordination “pathways” into the medical home model that improves health and social outcomes through the support of the community health workers (CHWs). The pathways model is developed as a strategy to track and improve accountability for positive, measurable changes as the CHWs help their clients work through specific health and social problems.

Financial evaluation of the cost effectiveness of accountable outcome production strategies are integrated with the Community health worker model in a multi-agency, community-wide intervention focusing on the most highly vulnerable individuals and families.1 (based on Dr. Mark Redding “Pathways”). The program includes care coordination throughout all settings including clinical and in school-based clinics.

Region-wide 3- Share Coverage Product for
Lower Income, Uninsured Employees

Core Objective
Develop a health coverage product for uninsured lower income employees that incorporates a 3-share model approach (employers, employees, and community sharing in the cost).

Region-wide Employee Wellness &
Consumerism Program

Core Objectives
Educate Central Oregon employers on best practices around health, wellness and medical consumerism in Crook, Deschutes and Jefferson counties. Provide a toolkit for employers to assist in the implementation of employee wellness and consumerism programs.

Develop region-wide employee wellness and consumerism program for small and medium employers.

Region-wide Community Chronic Disease
Management Program

Core Objectives
To continue an existing community-based chronic disease management program.

Expand the disease states within the program.

Expand outreach & enrollment in Crook, Deschutes & Jefferson counties.
