Bilingual SharedCare Community Links Specialist

REPORTS TO: Nurse Director, HealthMatters of Central Oregon
SUPERVISORY DUTIES: N/A
STATUS: Non-Exempt, Full time 1.0 FTE
COMPENSATION: This is a "Paid for Performance Position" $9.00 - $10.50 per hour DOE plus bonuses

The Bilingual Community Links Specialist will work with the Nurse Director, Medical Director, Enrollment Liaison and Outreach Manager to assist Spanish speaking clients enroll in SharedCare, navigate SharedCare services and improve their health.  Additionally, the BCLS will provide translation and interpretation support from staff and clients of all other HMCO programs as needed.  This position is a paid for performance position.  Community Links Specialists are compensated via bonuses upon completion of specified tasks and pathways.  The Pathways© Model employs community health workers who connect at-risk individuals to evidence-based care through the use of individualized care pathways designed to produce healthy outcomes. This model promotes timely, efficient care coordination through incentives and prevents service duplication through use of a Community Hub or network of agencies involved in providing care to the target population. 

ACCOUNTABILITIES AND REPONSIBILITIES:

  • Provide Spanish interpretation and translation services for SharedCare customers, staff and partners.
  • Use Pathways protocols to:
    1. Assist SharedCare staff in the enrollment of new SharedCare members.
    2. Assist SharedCare members in completing forms, answering inquiries regarding SharedCare plan, benefits and services.
    3. Provide support and guidance for Spanish speaking customers
    4. Educate clients about available community resources and refer clients to local social service agencies.
    5. Assist clients in utilizing resources, including scheduling appointments, and assisting with completion of applications for programs for which they may be eligible.
    6. Facilitate access to health related services, including but not limited to: assisting clients with obtaining a medical home, providing instruction on appropriate use of the medical home, assisting patients in overcoming barriers to obtaining needed medical care and social services.
    7. Provide Spanish speaking customers with education and social support; assist on-site nurse in providing health counseling and development of health improvement plans for Spanish speaking SharedCare members.
    8. Advocate for client and community to assure they receive needed services.
  1. Develop and maintain strong working relationships with referral sources and community agencies.
  2. Provide outreach to the tri-county Latino workforce regarding HealthMatters programs.
  3. Provide education to referral sources and community agencies as needed.
  4. Cultural mediation between communities and health and human services system.
  5. Empower clients to access appropriate health and wellness services.
  6. Translate and review forms, pamphlets and other materials for HealthMatters programs.
  7. Attend HMCO meetings and in-services and consult with HMCO supervisor on a weekly basis or more frequently as needed.
  8. Provide presentations to groups and individuals using power point; provide orientation for new SharedCare members
  9. Demonstrate a high level of commitment to the HMCO Mission, Vision, and Values.

KNOWLEDGE REQUIRED:

  • Knowledge or ability to acquire knowledge of the community being served and it’s residents, resources and problems
  • Knowledge or ability to acquire knowledge of federal, state, and local programs and their eligibility requirements and application processes
  • Knowledge or ability to acquire knowledge of principles and practices of Case Management
  • Knowledge or ability to acquire knowledge of health and social service systems
  • Knowledge or ability to acquire knowledge of health insurance or health coverage plans

EXPERTISE AND SKILLS REQUIRED:

  • Relationships with or connections to the community being served (a community member or possessing shared experience with community members being served)
  • Ability to establish and maintain effective working relationships with both clients and health care and social service providers
  • Motivated and capable of self-directed work; ability to work independently and manage own workload in a professional and responsible manner
  • Ability to prepare and maintain accurate records via a computer database
  • Ability to plan and administer work of a complex nature
  • Excellent communication skills, both verbal and written in both Spanish and English (medical interpretation/translation experience highly desired)
  • Ability to maintain confidentiality of client medical, personal and financial information
  • Strong desire to improve the health of our community\
  • Ability to learn programs and processes quickly
  • Proven ability to work closely in a team and communicate with a diverse group of individuals and groups
  • Professional demeanor and appearance

EDUCATION AND EXPERIENCE REQUIRED:

  • Minimum high school education or GED required; equivalent of two years college preferred
  • Minimum of 2 years experience in interpretation/translation for health, social or community services required
  • Bilingual Spanish/English fluency both written and verbal absolutely essential
  • Medical Interpreter certification required
  • Valid Oregon driver’s license and insurance required

HOURS & COMPENSATION: This position is a FTE position (40 hours per week). 

SALARY:  This is a "Paid for Performance Position" $9.00 - $10.50 per hour DOE plus bonuses. Benefits available after 90 probationary period.
